Reassessment: Tier / Formulation Selection + Questionnaire

Design the pre-payment reassessment steps: how a patient already on care selects a tier/formulation change (when applicable) and completes their reassessment questionnaire.

Today

No unified reassessment flow. Weight loss has a basic questionnaire path. No tier/formulation selection surfaced to patients. Non-WL products have inconsistent or missing reassessment.

Proposed

Single flow for all products. Matrix-driven tier/formulation selection with skip option. Dynamic questionnaire routing from Reassessment Links. Defined hand-off contract for the payment step.

Scope

Tier/formulation selection (including skip), questionnaire steps, error and edge states, hand-off data contract.

OUT

Payment step modifications (full renewal charge, delta-only tier change). Covered by a separate issue.

Product Scenarios

WL Injection (Tier 2)

Has upgrades + downgrades + formulation changes. Most complex scenario.

WL Oral (Tier 1)

Upgrades only (no downgrade from Tier 1). Oral reassessment questionnaire.

Non-WL (Skip)

No tier step. Straight to questionnaire or initial intake per Reassessment Links.
